When it happens to one of my teams I figure it's just a miscommunication or something from the sidelines that takes too long to figure out before the play clock expires. So a timeout is called. It happens once in a while even in real life. I haven't seen it a lot so I haven't been too alarmed.

I guess my teams' headsets work okay and haven't been messed with. :)
